Breasts may become more pendulous in all women in time. Where the nipple was once well above the crease under the breast (the inframammary crease), in time has settled lower. Pregnancy, breast feeding, and weight change or gain may all contribute to the breast's sagging more or becoming more pendulous.
A breast lift or mastopexy is an operation to lift the nipple and breast back above the inframammary crease. A breast lift or mastopexy can be combined with breast implants to increase or restore the volume of the breast or with a breast reduction (decrease in volume) to enhance the effects of the breast lift.

Depending on how much the breast has descended below the inframammary crease, there are different operations to lift the breasts and improve their contour. The surgery required to lift the breasts may be as simple as breast implants which re inflate breasts sagging after pregnancy, breast feeding, or weight loss. When there is a true excess of skin, a surgical pattern can lift the nipple and remove the amount of skin necessary to lift the breast and nipple. Sometimes both a breast implant and an excision and removal of excess skin is necessary. If the breast start out both large and sagging, it may be best to remove some breast tissue at the same time as raising the nipple and tightening the skin under the breast.
